Patient Instructions and Consent Form for Allergy Skin Testing
My doctor has explained to me why I need skin testing and what the procedure involves. I understand that there are 2 methods of skin testing: prick testing (also called scratch test) and intradermal (under/ within the skin) testing. One or both methods may be used for my testing. I will have the skin prick testing first. If the skin prick testing is negative, I may need intradermal testing to find out if there is an underlying allergy.
Benefits Allergy skin testing with help identify the things in environment and foods you are allergic to. I understand that there can be an alternative method of allergy testing, the ImmunoCAP (blood test) but this may not be as sensitive or accurate as skin testing.
Risk • Please let the physician and nurse know if you are pregnant or taking beta-blockers. Allergy skin testing will be postponed until after the pregnancy. Also, let the provider know if you have severe/ uncontrolled asthma and allergy symptoms which might put you at risk during skin test. • Reactions to the testing can include itchy hives (common), irritation, redness at the administration site which can last for several days or longer for individuals with sensitive skin, coughing and /or wheezing. • Anaphylaxis (severe allergic reaction), cardiac or respiratory arrest can occur but rare.
***Please note that severe reactions rarely occur but in the event a reaction would occur, the staff is fully trained; emergency medications and equipment is available. In the event of reaction, we may have to give you epinephrine, corticosteroids, antihistamines, oxygen and nebulized bronchodilators. If reaction is severe you may also need to go to emergency room. I will be observed for 30 minutes or longer after my skin testing in case of any reaction. If I have a reaction after I leave, I may call the office for advice or may go to the emergency room***
I have read the patient information sheet on allergy skin testing and understand it. The opportunity has been provided for me to ask questions regarding the potential side effects of allergy skin testing and these questions have been answered to my satisfaction. I understand that every precaution consistent with the best medical practice will be carried out to protect me against such reactions. I understand that it is important to tell my doctor about any health conditions or allergies that I have. I must also tell my doctor about any medications that I am taking.
